Track Checklist
Upon completion of this section, the following documents
should be placed in your F2P2 notebook:

Form 1: Documentation for Workshops - Six workshop hours (one form per workshop). Form 2: Event Attendance Form should be used to document your participation in a Teaching and Learning cohort (such as a Course Design Working Group) if you are counting those hours against your workshop hours.

Form 2: Event Attendance Form - Videotape consultation, classroom observation, or Small Group Analysis.

Form 2: Event Attendance Form - Faculty-supervised teaching and reflection.

Questions and responses for one interview with a Vanderbilt faculty member on teaching.
